PHQ-4: quick anxiety & mood check

When it helps: A 30-second front door: four questions that sample both anxiety and mood at once.

When it doesn't: It's a doormat, not the house. If it flags something, the fuller PHQ-9 or GAD-7 says more.

Four questions: two about worry, two about mood. It’s about the shortest honest screen there is: enough to notice a signal, not enough to explain it. If the number is higher than you’d like, the depression and anxiety questionnaires next door go into more detail. Like everything here, your answers stay on this page: nothing is sent, nothing is stored.

Four questions isn't much to go on. That's the point. It tells you whether it's worth looking closer, nothing more.
